I understand and agree that I am responsible for proper handling and the use of the Assistive Technology (AT) devices and all accessories that I borrow from The
Assisted Technology Training Center of CT. I understand it is illegal to copy or distribute any software loaned through this program.
I am responsible for returning all borrowed items, accessories, devices and components to The Assisted Technology Training Center of CT
By Click here to enter a date. (date). If I find that any components or items listed on the inventory sheet are missing when I obtain my AT Device loan, I must notify The Assisted Technology Training Center at 203-238-8362 immediately so I will not be held financially liable for the missing items.
In the case of loss of a device or components, I will contact The Assisted Technology Training Center at 203-238-8362 immediately.
The total replacement value of the borrowed item(s) equals: $ Click here to enter text. In the event of loss or theft, I will not be held responsible, as long as I immediately report the incident to the police, immediately notify The Assisted Technology Training Center at 203-238-8362 and provide a copy of the police report to The Assisted Technology Training Center at 203-238-8362. If an equipment breakage or malfunction occurs, I must immediately notify The Assisted Technology Training Center at 203-238-8362.
Failure to comply with these responsibilities will result in loss of future access to The Assisted Technology Training Center Program, in addition to possible financial liability.
RELEASE OF LIABILITY
I agree to indemnify and hold harmless The Assisted Technology Training Center and any and all employees, agents or representatives of same, from damages to property or injuries to myself, and/or any other person, and any other losses, damages, expenses, claims, demands, suits, and actions by any party against The Assisted Technology Training Center any and all employees, agents or representatives of same, in connection with loan(s) from The Assisted Technology Training Center Loan Program
